Community Organization Information Intake Form

Complete this form to share information about your community organization or mission-driven business in the Local Food Guide if you are located in the 11 counties we serve and work to support the production and distribution of local food or help to strengthen our region’s local food system.

The Local Food Guide was created to help promote and connect our region’s local food businesses and organizations. It was designed as a valuable resource for consumers, visitors, chefs and food service professionals across Northeast Indiana who want to know where their food comes from, how it is produced and who want to support local farmers and producers with their purchases.

Examples of community organizations and mission-driven businesses whose work supports our local food system include, but are not limited to: food banks who source food products from local farms, community gardens, organizations using locally sourced foods to improve healthy food access, food waste and composting programs, organizations and institutions that offer education and training related to local food systems, shared-use commercial kitchens, local food aggregation/distribution businesses, etc. We reserve the right to determine which businesses and organizations are listed in the guide and to verify the accuracy and authenticity of the listings.

If you need to change or update the information, you can either submit a new Information Intake form OR for a small change or if you have any questions regarding this form, email Ellen Bauman at Ellen@NEIfood.org.

After you submit your form, please email us one photo that represents your organization’s work OR your logo to include with your listing in the Local Food Guide. Please include “Local Food Guide Image” and the name of your organization in the subject line and email your image to Ellen@NEIfood.org.
